Waiver

• I agree to assume all risks arising out of my participation in any programs, activities or events at the Oakville Curling Club including, without limitation, the risk of personal injury.
• In making this application for membership, I accept these risks willingly, and I hereby release the Oakville Curling Club, Board of Directors, Management, Staff, Volunteers, Sponsors and Advertisers from any liability for any loss of property or injury or illness incurred while on the ice or while on any part of the premises.

• If my application for membership is accepted, I hereby agree to abide by the Club's by-laws, member code of conduct and rules and regulations at all times while a member thereof and pay annual fees applicable to my membership classification

• I acknowledge that unless I keep my personal belongings with me at all times there is a risk that they will be lost or stolen.

• I agree that I will not enter the arena until a signed Agreement has been received by the Club.

2021/2022 Special Assessment

• I agree to pay the Club's 2021/2022 Special Assessment fee, which will be billed by October 20, 2021 and payable by November 1, 2021 at a rate of $100 plus HST per member. I understand that this fee is non-refundable.
