• I agree to assume all risks arising out of my participation in any programs, activities or events at the Oakville Curling Club including, without limitation, the risk of personal injury.
• In making this application for membership, I accept these risks willingly, and I hereby release the Oakville Curling Club, Board of Directors, Management, Staff, Volunteers, Sponsors and Advertisers from any liability for any loss of property or injury or illness incurred while on the ice or while on any part of the premises.

Photo Consent

I hereby acknowledge that as a condition of my membership with the Oakville Curling Club (the 'Organization'), I consent to the capture and use of my image, likeness, and voice through photography or video during Club activities. I grant the Club a royalty-free, perpetual license to use these materials for promotional, social media, and historical purposes (including the Club website, Facebook, X, Instagram and YouTube). I waive the right to inspect the final product and release the Club and its directors from any liability regarding the use of these images. I understand that checking this box is mandatory for registration; should I have exceptional privacy concerns, I agree to notify the Club Manager in writing immediately.

Photo Consent

As the parent or legal guardian of the registrant, I hereby grant the Oakville Curling Club ('OCC') permission to photograph or video my child during Youth Progamming, U18/U15/U12 events, or other youth-related events. I agree that these images may be used in OCC newsletters, on the Club’s official social media accounts, and in promotional materials to celebrate youth curling in Oakville. I understand that no last names will be used in social media captions without additional explicit permission. I acknowledge that this consent is a requirement for program participation. I release the OCC from any claims arising from the use of these materials and represent that I have the legal authority to provide this consent.

The Oakville Curling Club is a community-driven organization. We use photos of our leagues and bonspiels to celebrate our members and grow the sport. This mandatory consent allows us to share the spirit of the club safely and legally.

If you wish to withdraw media consent for a specific reason after registration, please contact the Club Manager. We will make every effort to remove digital images, though we cannot recall printed materials already in circulation.

2021/2022 Special Assessment

• I agree to pay the Club's 2021/2022 Special Assessment fee, which will be billed by October 20, 2021 and payable by November 1, 2021 at a rate of $100 plus HST per member. I understand that this fee is non-refundable.
