Waiver
- I understand that my Parent/Guardian is required to sign a copy of OCC's Informed Consent and Assumption of Risk Agreement (U-18) ("Waiver"). (Click to e-sign)

Club Policy
- If my application for membership is accepted, I hereby agree to abide by the Club's by-laws, member code of conduct and rules and regulations at all times while a member thereof and pay annual fees applicable to my membership classification
- I agree to and accept the Club’s Refund Policy.
Other Acknowledgements
- I understand that The Oakville Curling Club's main form of communication with members and participants is by email.
- I acknowledge that unless I keep my personal belongings with me at all times there is a risk that they will be lost or stolen.
COVID-19
- I understand that COVID-19 has been declared a global health pandemic by the World Health Organization and that Club health and safety protocols may change based on the prevailing pandemic conditions and/or public health requirements/recommendations.
- I understand that these changes may include, but are not limited to, mandatory masking and vaccination requirements, and agree to abide by all health and safety protocols while participating in Club activities.
